Abstract :

Aortic dissection is a usually acute, fatal disorder in a vascular condition. The table is usually presented with an acute mortality rate of the disease of more than 50% in the first 48 hours. While we see the various tables clinically encountered, with chest pain is the most common prey. Also, according to the region involved, the aorta may be associated with conditions, such as myocardial infarction, syncope, renal failure, and paraplegia. As aortic aneurysms may develop during the course of dissecting, patients in the course of chronic aortic dissection in 25% of aortic aneurysms can be seen. Especially, ascending and/or aortic arc involvement and acute and chronic condition of disease are potentially fatal. Therefore, the most important point in the diagnosis is clinical suspicion. In this article, a huge aortic aneurysm and thrombus, developed in the presence of asymptomatic chronic aortic dissection, are discussed. In conclusion, aortic dissection and/or aneurysm should be kept in mind in the etiology of ischemic cerebrovascular events in the hypertensive and elderly patients. Genellikle akut bir tablo ile karsimiza cikan hastaligin mortalitesi ilk 48 saat icerisinde %50'dir. Klinik olarak degisik tablolarla karsimiza cikabilse de en sik yirtici vasifta gogus agrisi ile karsilasilmaktadir. Ayrica aortta tutulan bolgeye gore miyokard enfarktusu, senkop, bobrek yetersizligi, parapleji gibi durumlarla da birlikte olabilir. Hastalik nadir olarak da asemptomatik kronik bir seyir izleyebilmektedir. Aort anevrizmasi seyri sirasinda diseksiyon gelisebilecegi gibi, kronik aort diseksiyonu seyrinde hastalarin %25'inde aort anevrizmasi gorulebilmektedir. Ozellikle cikan ve/veya arkus aort tutulumunda hem akut hem kronik durumda hastaligin mortalitesi yuksektir. Bu nedenle hastaligin tanisinda klinik suphe en onemli noktayi olusturmaktadir. Bu yazida asemptomatik kronik aort diseksiyonu zemininde gelisen dev aort anevrizmasi ve trombus saptanan bir olgudan bahsedilmektedir. Sonucta; ileri yasta hipertansif hastalarda iskemik serebrovaskuler olay etiyolojisinde aort diseksiyonu ve/veya anevrizmasi akilda bulundurulmalidir.
